Description
The Community Ageing Well Service (CAWS), formerly Frailty Multi-Disciplinary Team (MDT), supports Barnet patients aged 65+. The NHS Core Frailty MDT Team is based at Edgware Community Hospital, CLCH. This contract is to fund the provision of a Frailty Multi-Disciplinary Team (MDT) Coordinator to work with the NHS, Age UK Barnet and other organisations, in support of NHSE's Ageing well/Living Well programme.
