Aberdeen City Health & Social Care Partnership wish to engage a social care provider to develop and deliver a Discharge to Assess Test of Change. This will be run over a maximum period of two years, with built in monitoring and evaluation to ensure that outcomes are being met. The Test of Change will be delivered by a dedicated team who would work alongside hospital staff in Aberdeen Royal Infirmary to identify individuals who are medically fit to be discharged but require a period of recovery, assessment and enablement in their own home environment.
