Description
The Leeds Electronic Market Place (EMP) shall provide citywide Short Breaks for children and young people with SEND who ordinarily reside in the Leeds boundary. All children and young people accessing the services have an Education, Health & Care Plan (EHCP) or are waiting for an assessment. Lead Professionals, Social Workers, and parents/carers will refer children and young people to the Local Authority Short Breaks Panel which will assess each case and if approved, will allocate a service that can best meet the children and young people's needs but also give a choice where possible…
