The City of York Council Public Health service are seeking to engage with suitable suppliers to procure access to a NHS Health Check service for eligible citizens aged 40-74. To be eligible for an NHS Health Check, citizens must live within the City of York Council area, be aged 40-74. Those with an existing cardiovascular or metabolic condition (e.g. diabetes or high blood pressure) are excluded, as the aim of the programs is to find undiagnosed and treatable conditions.
